The CHC and Doctors on Duty (DOD) care about you, your ideas, and concerns. Please let us know if you have comments or suggestions that can help us to continue meeting your health needs or improve our medical services.

Comments, Complaints, and/or Suggestions can be submitted by phone, email, or in person.  All are reviewed by the Clinic Administrator.  When authorship or identity is given, the Administrator and/or the Doctors on Duty (DOD) Supervisor will make contact and address the issue, whenever possible. 

Note that the CHC provides Comments/Suggestion forms in the Lobby and each Exam Room - when filled out, those completed forms can be left with any staff member or dropped in the Suggestion/Comments Box located in the front Lobby.

Patient Questionnaires/Satisfaction Surveys are taken twice a year; one (1) week in February and   one (1) week in October.  The anonymous survey forms are available in each Exam Room and at the Front Desk and provide opportunities for patients to help us improve service, make comments and  suggestions, and recommendations.

We appreciate your input.  However, if/when you call, please remember that we may be serving other patients and might have to call you back.
